Estate Planning for PWD

I would like to suggest a new forum for estate planning and legal issues for PWD. I am in the process of preparing various legal documents, Living Will, Power of Attorney, Health Care Proxy, etc.
I am curious as to what specific end of life care issues should people living with diabetes be considering or concerned about?
For example should there be any specific directions about diabetes care?
Any comments, suggestions or recommendations are welcomed. I would like to hear from those living with diabetes who have been through the process and have these legal documents as part of their estate plan. Thanks.
